A review of my past payment record with you would show that I have always made my payments on time.  These few late payments were the result of (lost job, unexpected illness, etc.) which forced me to temporarily fall behind paying my debts.  However, I recovered quickly and resumed making all payments on time.

For this reason, I am requesting that you contact [name of credit bureau] and have these late payment notations deleted from my credit report.  If it weren't for these few late payments, I would have an excellent payment history with your company and do not believe it is fair for my credit rating to suffer because of a temporary situation.
